Have what on it.....am I missing something...? That looks like what is commonly called a 'tuner nut/bolt'.

These are used when a wheel wrench won't go over the end of a standard nut due to the size of the hole surrounding it on the alloy. They unscrew by the use of a adapter that goes in the middle hole. Fairly common.
